The Educare Center of Milwaukee is focused on developing a partnership with each family. The first opportunity to develop this partnership is at enrollment. Early Childhood Family Engagment Specialists work with individual families to complete this process. 

To begin, families must first complete a pre-application. The Pre-application contains information about your child and family that will help us determine if your family is eligible for the program. Click here to view and print our pre-application form, fill it out and bring it with you when you visit the center.

The following documents must accompany the pre-application to help us determine if your child can be enrolled:

• Income Verification
• Verification of Residence
• Child’s Birth Certificate
• Child’s Social Security Card
• Current Immunization Record
• Dental Examination Record
• Medical Insurance/Badger Care Card
